"I agree to give permission to Markham, Richmond Hill & Vaughan Chinese Business Association for the free use of my name and picture in any media including, but not limited to, any type of broadcast, telecast, newsletters (both electronic or paper), newspapers and program books etc. I am also aware and agree to assume all the risks by myself for participating in all the events arranged by the Association. I further agree not to make any claims whatsoever against the Association, its directors, or employees for any loss, damage or injury incurred."
